Making An Appointment
A physician referral can be faxed,
telephoned , or mailed to our clinic.
We request that the patients medical
records, imaging studies and laboratory work be mail to us for review prior to the
patient’s visit.

health insurance plans. In some cases, your insurance company may request a referral from
your primary care physician.
